Are you a new patient?
If you are a new patient, please make sure when scheduling your appointment that you are sent an email with the patient registration forms.
You will be asked to sign for these forms, in office.
Have not received patient registration forms?
If you haven’t received your patient registration forms, please be sure to check you spam and junk folder. If then, you still haven’t received the patient registration
forms, then please contact our office. You may call or text our office number, stating that you have not received your forms. We will confirm your email and resend your forms as
needed.
Having trouble with completing the patient registration forms?
If you are having trouble with completing the forms, please give us a call and our front desk would be more than happy to assist you, with any questions you may have.
Are you a returning patient?
If you are a returning patient, welcome back!
Please be sure, when scheduling your appointment that you mention, you’re an existing patient. Make sure that you receive an email to update your forms
on any changes. (ex: changes on health, new medications, etc.)
Returning patients with no changes within their health or changes in general, you will still be sent the email. Please be ready to sign in office.
